One set that we can keep with us at the medical station during camp week. One set that the camper can give to their coach to hold during the day, but is given back to them at night to take home. The second set that we hold is a spare set in the event that the camper leaves his\/her emergency medication at home.<\/span><\/p>\n<div><span style=\"text-decoration: underline;\"><span style=\"color: #333399;\"><strong>Specific Medical Condition Action Plan Forms:<\/strong><\/span><\/span><\/div>\n<ul>\n<li>Campers who have severe Allergies, or suffer from Asthma, Epilepsy, or Diabetes will need to complete and submit the appropriate Action Plan Form.<\/li>\n<li><a href=\"https:\/\/coachwootten.com\/site\/wp-content\/uploads\/2014\/10\/Allergy-Action-Plan.pdf\" target=\"_blank\" rel=\"noopener noreferrer\">Allergy Action Plan<\/a><\/li>\n<li><a href=\"https:\/\/coachwootten.com\/site\/wp-content\/uploads\/2014\/10\/Asthma-Action-Plan.pdf\" target=\"_blank\" rel=\"noopener noreferrer\">Asthma Action Plan<\/a><\/li>\n<li><a href=\"https:\/\/coachwootten.com\/site\/wp-content\/uploads\/2014\/10\/Diabetes-Action-Plan.pdf\" target=\"_blank\" rel=\"noopener noreferrer\">Diabetes Action Plan<\/a><\/li>\n<li><a href=\"https:\/\/coachwootten.com\/site\/wp-content\/uploads\/2014\/10\/Seizure-Action-Plan.pdf\" target=\"_blank\" rel=\"noopener noreferrer\">Seizure Action Plan<\/a><\/li>\n<\/ul>\n<p><strong><span style=\"text-decoration: underline;\"><em>Concussion Awareness FACT sheets:<\/em><\/span><\/strong><\/p>\n<p>Basketball is a very physical sport and can result in a participant sustaining a concussion.
