Please submit forms to us via <a href=\"medical@coachwootten.com\">email<\/a> or fax (1-866-524-3059) 10 days before your first camp session.<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p><span style=\"text-decoration: underline; font-family: arial, helvetica, sans-serif;\"><span style=\"font-size: 14pt;\"><em><strong>Medication Policy<\/strong><\/em><em><strong>:<\/strong><\/em><\/span><\/span><\/p>\n<p><strong><span style=\"color: #ff0000;\">Campers are not permitted to keep any medications including prescription, over-the-counter, vitamins, and topical creams and ointments on their person or in their dormitory room.<\/span><\/strong><\/p>\n<ul>\n<li>If you have a camper who requires daily medication or occasional over-the-counter medication, please check in with the medical staff at registration check-in.<\/li>\n<\/ul>\n<ul>\n<li>If your child has a food allergy or sensitivity, we will put you in contact with <a href=\"Pamela.Detrick@compass-usa.com\">Pamela Detrick, Dining Hall Services<\/a>, and you can discuss the menu provided in the Dining Hall. FSU dining hall is a NUT-FREE environment. However, the campus is NOT a NUT-FREE environment.<\/li>\n<\/ul>\n<p><span style=\"text-decoration: underline; font-family: arial, helvetica, sans-serif;\"><span style=\"font-size: 14pt;\"><em><strong><span style=\"color: #000000; text-decoration: underline;\">Prescription or Non-Prescription:<\/span><\/strong><\/em><\/span><\/span><\/p>\n<ul>\n<li><span style=\"font-size: 12pt; font-family: arial, helvetica, sans-serif;\">Campers who take over-the-counter or prescription medication must submit a Medication Administration Authorization form mandated by the state of \u00a0Maryland. <span style=\"color: #ff0000;\">Please note only prescription medications need to have a physician\u2019s signature. Over-the-counter medications only need to have the<\/span> <span style=\"color: #ff0000;\">parent\/guardian\u2019s signature<\/span>.<\/span><\/li>\n<\/ul>\n<ul>\n<li><span style=\"font-size: 12pt; font-family: arial, helvetica, sans-serif;\">Download the <a href=\"https:\/\/coachwootten.com\/site\/wp-content\/uploads\/2014\/10\/MedicationAdministrationAuthorizationForm-1.docx\">Medication Administration Authorization Form<\/a>\u00a0here.<\/span>\n<ul>\n<li>All medication must be in the <em><strong>original container<\/strong><\/em> with the camper\u2019s name and DOB on the prescriptive label. We are not permitted to accept medications in a weekly\/daily generic pill dispenser.<\/li>\n<li>Please bring <em><strong>two sets of emergency medications.<\/strong><\/em> One set will remain with the camper.
